Cancellation Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the providers day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee to the card on file.
Health & Wellness Coaching & Nurse Practitioner Service Agreement
1. Scope of Services
The Provider agrees to offer health and wellness coaching or nurse practitioner services to support the Client in improving their health, well-being, and lifestyle. This may include, but is not limited to:

Health coaching and goal-setting

Lifestyle and behavior modification support
-
Nutrition and exercise guidance
-
Preventive health education
-
Nurse practitioner consultation and treatment (within scope of license)

This does not include chronic condition management
Goals and Objectives:
The primary goal of this agreement is to assist the Client in achieving improved health outcomes through education, planning, and supportive care. Specific goals will be determined collaboratively in the initial consultation.
Expected Outcomes:
While outcomes may vary based on individual effort and circumstances, the Provider commits to delivering Christ-centered evidence-based guidance, encouragement, and support throughout the agreed-upon coaching or clinical sessions.
Confidentiality and Privacy
All information shared between the Client and Provider will remain strictly confidential, in accordance with federal and state laws (including HIPAA, where applicable). The Provider will not disclose any personal or medical information without the Client’s written consent, except where required by law (e.g., danger to self or others, court order, mandatory reporting).
Roles and Responsibilities
Provider agrees to:

Provide individualized coaching and nurse practitioner services to the best of their ability.

Maintain appropriate licensure and certification.

Communicate clearly and promptly.

Protect the Client’s privacy and maintain confidentiality.
Client agrees to:

Participate actively in coaching and care sessions.

Be honest and open during sessions.
-
Follow agreed-upon action steps and recommendations.

Communicate any concerns or changes in health status.
4. Session Logistics and Fees

Session Frequency and Duration: Sessions will occur weekly/biweekly/monthly and last approximately 30/60 minutes based on Client preference.

Location: Sessions may take place in person, via phone, or via secure telehealth/video conferencing platforms.

Fees: The Client agrees to pay $125 for the initial coaching session and $90 per additional sessions or a package rate of $900 for 12 sessions. The Client agrees to pay $150 for the initial nurse practitioner session and $100 per additional sessions or a package rate of $1,000 for 12 sessions.

Payment is due prior to session.
5. Cancellation, Termination, and Refund Policy

Cancellation Policy: The Client must provide at least 24 hours’ notice for cancellations. Failure to cancel within this window may result in a missed session fee.

Termination: Either party may terminate this agreement at any time with written notice.

Refunds: Prepaid sessions may be refunded on a prorated basis if the agreement is terminated early. No refunds will be issued for completed sessions.
6. Disclaimer
The Provider is not a substitute for emergency medical care, psychological counseling, or specialist services. Health coaching sessions should be used in conjunction with primary medical care.
